If a class or student desires to administer a survey outside the University, the department chair under whose jurisdiction the project falls shall submit to the Vice President of Academic and Student Affairs at least one week prior to beginning the survey a copy of the survey, together with any pertinent information, including: 1. To whom the survey is to be given; 2. What is the purpose of the survey; 3. Name of the faculty member involved; 4. How the results are to be evaluated and used; and 5. The date survey is to be administered. If no response is given to the department chair by the Vice President of Academic and Student Affairs prior to the date the survey is to begin, it may be assumed that approval has been gained. |
